    Info Request

At LifePath Hospice, we go way beyond those basic hospice requirements to identify and meet the specific needs of each patient and family—from extra homelike touches in our two hospice houses to the hands-on bereavement care we provide to families long after their loved one has died.

When you ask for us by name, ask about our quality, compassionate care that goes far above and beyond the basic hospice requirements.

LifePath Hospice Mission Statement - The mission of LifePath Hospice is to provide quality palliative care and relieve the suffering of those in our communities affected by life-limiting illnesses and end-of-life issues, maintaining the highest ethical standards, so all may live as fully and comfortably as possible.

As a Not-for-Profit Provider, LifePath Hospice we never deny services to anyone due to financial hardship. Patients receive services wherever they call home—in a primary residence, assisted living facility, skilled nursing home, hospice house or hospital.

We accept Medicare, Medicaid, Tricare and most private insurances. Under Medicare, Medicaid and Tricare, hospice is an all-inclusive benefit—all services, medications, home medical equipment and medical supplies related to the patient’s illness are covered.
