As an alternative to residential nursing facilities, InnovAge PACE (Program of All-inclusive Care for the Elderly) helps seniors keep living at home. Our neighborhood care center provides a wide range of healthcare and social services and benefits all in one place.

Are you eligible?  To participate in InnovAge PACE you must be:

• At least 55 years old

• Able to live safely at home and in the community with support

• Eligible for nursing facility level care

• Living in an InnovAge PACE service area

The support you need, all in one place!  Our neighborhood care center provides a wide range of healthcare and social services and benefits to qualifying seniors.

• Coordinated Healthcare - Primary, emergency, and hospital care

• Social Engagement - activities, meals, exercise, and group outings

• Approved Transportation - rides to medical appointments and centers

• In-Home Assistance - help at home, including equipment and suppies

• Medication management - prescription and OTC meds with no co-pay

• Specialty Care - dental, vision, hearing, foot care, physical and speech therapy, and behavioral health

How much does it cost? 
Most seniors enroll at little or no cost with Medicare and Medicaid.  If you are:

• Eligible for Medicaid: No Cost*
• Eligible for Medicare & Medicaid: No cost*
• Eligible for Medicare only: Pay Medicaid portion, plus monthly premium for Medicare Part D (prescriptions)
• No eligible for Medicaid or Medicare: Self-pay rate

* If you live in or move to supportive housing (assisted living or nursing home), there is a co-pay to that facility. Also, some states require a share of cost fee if your income exceeds the Medicaid threshold.

Does anyone live at an InnovAge PACE Center?  No one lives at an InnovAge facility. While InnovaAge provides full, wrap-around, coordinated healthcare, the seniors enrolled in PACE live in their own homes or in supportive housing based on their individual needs.
