    We believe that individuals and families are experts in their own care, which is why we offer a personalized plan for care for each patient and family we serve. Our community-based organization consists of a team of trained hospice and palliative care professionals that are committed to helping our patients and their loved ones live each day to the fullest.

    We provide care to our patients wherever they call home, whether that is a private residence, nursing home, Assisted Living Facility, or other location.  We understand that each patient and family’s journey and needs are different. As a Medicare-certified hospice, AccentCare is proud to offer all four levels of hospice care: Routine, Inpatient, Respite, and Continuous Care. We work with our patients and medical team to determine the level of care appropriate for each patient.

    Open Access  - Open Access is a Hospice program that allows patients currently receiving medical treatment and or experiencing intense psychosocial issues access to Hospice services earlier; A blended model between curative and palliative care

    Music Therapy - Sometimes when a patient is feeling sad, distressed, or in pain, hearing a familiar song from their youth or that that brings back a particular memory seems to have a soothing effect. In fact, this is proven by science seasons provides music therapy to all of our patients who desire it because of the comfort and support that we’ve seen it provide again and again.

    Namaste Care – namaste care involves meaningful sensory activities to stimulate the senses, promote relaxation, and offer comfort and serenity to our patients. It is designed to make patients feel valued and seen, and can enhance the lives of people of any age with any disease, as well as providing moments of peace and tranquility to family caregivers. It is often particularly meaningful for patients with dementia.

    Psychosocial Support – during a patients end of life journey, complex feelings, emotions, and relationship dynamics often arise. Our credentialed social workers and chaplains support patients and families throughout this journey, providing resources and support that are customized to each person and situation.

    Bereavement Support – staff at our call center are available 24 hours a day, seven days a week, including on holidays. It’s important that our patients and families can always get in touch with us right away. Our registered nurses and customer service representatives are always available to provide clinical advice or emotional support, put caregivers in touch with a member of their local care team, schedule a visit, or answer any questions about a care schedule. Our call center staff have access to the same electronic medical records that all of our Hospice team members use. This allows them to look at the patient’s medication list and the notes that a care team have enter after visiting a patient in person, so our representatives can provide up-to-date and immediate assistance over the phone.
